Name and address of your:

Supplier/manufactuer

The company or person that manufactured the goods or last modified them.

Vendor

 The company/person who sells the goods.

Buyer

The person or company purchasing the goods.

Consolidator

The person or company purchasing the goods.

Shipment destination

The item’s final destination.

Container stuffing location

Location where goods were loaded into the container. (If this was more than one place, list all the locations.)

HTS codes

The Harmonized Tariff Schedule numbers with a minimum of 6 digits.

Bill of lading number

 Either the house or straight bill of lading number. Choose the lowest bill.
